The Chalet at Glarner Village Post-Acute Nursing & Rehabilitation, physical therapy, occupational therapy and speech therapy are available in an inpatient setting. The medical director leads the team in developing an individualized treatment program designed to help achieve optimal independence and to prevent further decline in function.
Physical Therapy services enable individuals to restore and/or achieve optimal movement and function and to relieve pain. Through evaluation and treatment programs, the physical therapist can treat persons with a variety of ailments including:
Balance / gait disorders
Low back and neck pain
Other orthopedic / athletic injuries
Treatment following joint replacement surgery
Occupational therapy services enable individuals to achieve independence in their lives despite their disabilities. The Occupational Therapist facilitates, promotes, maintains and enhances performance in activities of daily living skills regardless of physical, developmental, social and emotional problems. Services include:
Retraining in daily living activities
SPEECH AND LANGUAGE PATHOLOGY
These services enable individuals to maximize functional speech, language, cognition and swallowing skills. Services are provided to residents of all ages who have a range of diagnoses such as:

Preventative and Proactive Care
We feel that smooth care transitions are a crucial part of a sound care program so partnering with primary care physicians are essential for personalized care coordination. Effective communication with residents and their loved ones about advanced care planning, day-to-day needs and changes is a large part of what we do - these conversations are led with help of the primary care physician, our Nurse Practitioner and the Interdisciplinary Team. At New Glarus Home, education is a priority. We feel that through education of staff on topics and conditions that affect our residents and their families, that we can provide a culture of care that goes above and beyond addressing the physical needs, but can enhance the residents overall choice and well-being.
Innovative and Effective Care
New Glarus Home focuses on improving the quality of care for our residents through effective transitions among levels of care. This includes transitions to and from other levels of care and the hospital. We feel that it is essential to reduce the amount of avoidable hospitalizations that can cause disorientation, higher frequencies of infection, and additions of unnecessary medications. This is done through communcation with primary care providers and through education of staff at New Glarus Home. All of these things contribute to fewer impatient admissions, readmission's and emergency room visits for the residents.
